The primary purpose of completing the W-9 is to provide official verification of name, address, and taxpayer identification number (TIN) for employment and other income-producing purposes. So, in some cases, freelancers or other independent contractors are required to complete printable blank W-9.

It is important for employers who report their financial transactions to the Internal Revenue Service (IRS). The W-9 form itself is very small, with only a few fields (1-7), and two parts with boxes for TIN-number and your signature. However, the current version of this form consists of 6 pages, as it provides detailed instructions for filling it out correctly with a description of all the nuances. If this instruction is too big for you, we will describe it more briefly here, since freelancers and entrepreneurs do not need to understand all these details.

How to get a W-9?

There are 3 main ways to obtain the blank:

How to properly fill out the W-9 form and whom is it required?

  • The form must be completed by U.S. citizens or foreign residents who live and earn income in the United States.
  • The form is filled out in English, and mistakes cannot be corrected in it. If you do need to make a correction, you must print the W 9 blank again and fill it out.

Filling out items 1 through 7 on blank W-9 online

  1. Enter your Full Name here, as you indicate it on your tax return.
  2. Enter your company name or the name under which you make business. That is your business or legal name for your customer or marketing service.
  3. Specify the legal status of your organization. That is, this is where you need to specify if you are:
    • Partnership;
    • Trust or estate company;
    • C Corporation or S Corporation;
    • LLC company (Limited Liability Company);
    • Individual, single LLC member, or sole entrepreneur;
    • Or another organization type.
  4. Enter the correct payee code in the box if they have certain tax exemptions.

You can probably leave these fields blank, as individuals, freelancers, and self-employed people do not have it. But there are some organizations that do have such benefits. In most cases, they are trusts funds, financial institutions, and corporations, so this box is for them. A list of these codes (1 through 13) is on page 3. If your company is a corporation, for example, enter ‘5’ according to the list.

But beyond that, there are also some types of payments that may be exempt from withholding. They are listed in the table, which you may find on page 4.

In addition, there is another field in the box that contains the reporting exemption code (from FATCA – the codes from ‘A’ to ‘M’, depending on the type of your organization and listed on page 4).

But freelancers and any independent contractors are not exempt from this reporting, so this box is probably not for you.

  1. Provide the address (house number, street number, apartment number). For example – 700 Carpenters Crossing, apt 25.
  2. Enter your city, state, and zip code. For example – New York, NY 11256.
  3. Provide the list of your accounts (optional field). It is not necessary to list all account numbers here, as it may limit this form.

Part 1. Providing Taxpayer Identification Number (TIN).

If you are an individual or sole proprietor, then you should enter the Social Security Number (SSN) in this section. However, foreign residents may not have this number, in which case you must enter an individual taxpayer identification number – ITIN, or EIN (Employer Identification Number) in the appropriate field below.

Note that the TIN (U.S. Taxpayer Identification Number):

  • must consist only of digits, which must not be more or less than 9;
  • must not have 9 of the same digits or 9 consecutive digits (ascending or descending).

If you do not have a TIN, but you intend to apply for one or have already applied for one, indicate ‘Applied for’ in the ‘Social Security Number’ or ‘Employer Identification Number’ box. Then you will have 60 days to provide your TIN to the Office.

Part 2. Completion of form W-9

If you are a legal resident of the United States, please put your signature and current date in this box in the appropriate format (month, date, year).

If you are signing this on behalf of a legal entity, you must certify that you are authorized to sign it on behalf of one.

Enter your legal name on line 1 as it’s shown on your Income Tax Return.

Use trade name on item 2, in case it’s different from the legal name.

Choose entity type. If you’re LLC, also mentioned the tax classification of Limited Liability Company (S Corporation, C Corporation or P Partnership).

4.

Use exemption codes, in case your business is exempt from backup withholding and/or FATCA reporting.

5.

Enter full address, city, state and ZIP code on lines 5 and 6.

6.

Use your TIN on Part I. It’s SSN for individuals, EIN for companies and ITIN for residents alien.

7.

Sign the form and put a current date. If you’re a subject to backup withholding, cross out item 2 in part II
